To assess the role of prolonged anti-estrogen use for the risk of breast cancer in postmenopausal women. The current study is based on the Nurses' Health Study (NHS) I and NHSII cohorts. In NHSI, we interviewed 2209 women in 1976-1981 regarding reproductive and menstrual history, oral contraceptive (OC) use, and other risk factors, and in NHSII we interviewed 7731 women in 1990-1996 regarding these factors. Participants provided medical information and a blood sample, and breast cancer incidence was ascertained through self-report and confirmed by medical record linkage. During the follow-up of the NHS I cohort through 1998, 676 incident cases of breast cancer were diagnosed. Compared to women who never used OCs or who used OCs for less than 2 years, women who used OCs for longer than 2 years had no significant risk of breast cancer [relative risk (RR), 1.01; 95% confidence interval (CI), 0.80-1.27]. Similar results were observed when the analysis was restricted to women who ever used OCs (RR, 1.03; 95% CI, 0.82-1.29). The RR for women who used OCs and were postmenopausal at the start of OC use was 0.78 (95% CI, 0.56-1.09) compared to women who were premenopausal at the start of OC use.
